Software Errors that cost millions

The Mars polar lander went missing in 1999. They investigated, found out that the software likely mistook the jolt of the landing grear extending for the jolt of hitting ground, turned off the rockets, making the craft plummet from 40 meters and smash to smitherines on impact.
